This is a tricky biopsy. Getting into the S1 body can be done using an angulated transsacral approach or sometimes using a posterior approach or a decubitus approach.
What is important is to understand the anatomy to avoid the nerves and thecal sac. The video explains how I arrived at the approach after studying the anatomy.
